7th Grade Immunization Requirements

All students entering the 7th grade will need a Tetanus booster to complete their school immunization requirements.  This is a state law and each student will need documentation before 7th grade admission.

On May 7th and 8th, the Escambia County Health Department will be at Ransom Middle School to administer the tetanus booster to 7th graders at no cost. 

Each parent will receive an information sheet and consent form the first week in April.  Please return the consent form upon receipt to assure that your child receives the immunization.  Immunizations cannot be administered without consent.  Below is a link to the consent if your child has "lost" it.

THE SCHOOL BOARD OF ESCAMBIA COUNTY

HEALTH REQUIREMENTS FOR REGISTRATION

 

PHYSICAL EXAMINATION

 The school board of each district and the governing authority of each nonpublic school shall require that each child who is entitled to admittance to pre-kindergarten or kindergarten, or is entitled to any other initial entrance into a public or nonpublic school in this state, present a certification of a school-entry health examination performed within 1 year prior to enrollment in school.  F.S. 1003.22 (1) ·        Therefore, any new student entering Florida schools for the first time, including pre-kindergarten or kindergarten, should present a certification (DH form 3040, 6/02) of school-entry health examination by the first day of school attendance. However, under hardship circumstances, a new student may be given a 30-day grace period when necessary to obtain school physical.  Documentation must be provided by 30 days, or the student will be temporarily excluded from school pending documentation of the exam.·        Health examinations performed by licensed health professionals outside of Florida are acceptable as long as the documented exam was completed within 1 year of the first day the student will attend school. 

IMMUNIZATIONS

 The school board of each district and the governing authority of each nonpublic school shall establish and enforce as policy that, prior to admittance to or attendance in a public or nonpublic school, grades preschool through 12, each child present or have on file with the school a certification of immunization (DH form 680) for the prevention of those communicable diseases for which immunization is required by the Department of Health.  F.S. 1003.22 (4) ·        The parent is responsible for assuring that their child is in compliance with the immunization law. F.S. 1003.22 (7)·        A student who is not in compliance with the immunization law will be refused admittance and/or temporarily excluded from school attendance. F.S. 1003.22 (10) (a) (b)·        Up to a 30-day grace period may be given for new students to Escambia County to obtain records of immunizations in order to provide a FL Certification of Immunizations. EMERGENCY HEALTH CARDS An emergency information card, updated annually, shall be completed for each student listing contact person, family physician, allergies, significant health history, and permission for emergency care. 64 F-6.004 Florida Administrative Code·        Emergency Card (9400-HES-003) must be completed in its entirety with signature.·        Parents of uninsured students are encouraged to apply for FL KidCare, a high quality,low cost health insurance for uninsured children through age 18 years.   NO REGISTRATION IS COMPLETE WITHOUT THE ABOVE HEALTH FORMS.   HEALTH RECORDS ARE ACCEPTED ON TEMPORARY BASIS PENDING DOH REVIEW AND APPROVAL.
