Physical Examination

Upon entry, all Readiness students are required to have a physical examination form on file. The date of this form must be within one year of entry, and will be due again annually on that date. You will be sent a notice approximately one month before the next due date.
For physicals "due" over the summer months 0f 2011, notices were sent out May 17.
Dental Examination

All Students in the Readiness Program are required to have the name, address and phone number of their dentist on file. We encourage a dental examination prior to entry.
Health History

All Readiness students are required to have a Health History on file. This form is to be completed by a parent and should be returned the first day of school.
