New Requirements
Last updated May 23rd, 2011 at 6:01 a.m.In August of 2010 the following vaccination changes were implemented. The following information is current for the 2011-2012 school year:
-Children entering Readiness at 5 years of age (Kindergarten year) are now required to have two varicella vaccinations (chicken pox) instead of the one previously required.
-Students entering grade 7 must have one dose of Td (Tetanus booster) or Tdap (Tetanus/Pertussis booster) prior to entry. If they have had either shot within the last 2 years, that date will be acceptable. Unless your child has had a serious injury, they most likely have not had this addtional immunization (since the series required to enter school) and will need the booster. When your child receives the Td or Tdap booster, please complete the information below, print the page and send to school to my attention (or use the hard copy form sent home in May 2011. I will need this information by the start of school in August. Please send now if already completed.
