General Information; including new student physical exam form
Dear Parent/Guardian,
Welcome to Alloway Township School! I would like to take this opportunity to welcome you and to inform you about the medical requirements that will need to be met before your child enters school.
Medical Examination (form is at the end of this page) –
- If your child is entering a public New Jersey school for the first time, a medical examination is required and the attached form must be completed before entering school in September (or submitted within 30 days from the date of enrollment if registering after school starts).
- If your child has previously attended a public or private school, I will review your child’s records when they are received from the previous school attended. If your child had a previous medical examination and the appropriate documentation is provided, your child will not need another physical. If documentation is not provided, I will contact you and you will then be given 30 days to have the examination completed and the form submitted to my office.
Immunization Record –
You will also need to provide a copy of your child’s immunization record. This record must be provided and reviewed by me before your child enters school.
