General Immunization Requirements

Below are minimal immunization requirements established by the New Jersey Department of Health for the various grade levels. Please feel free to contact me regarding more specific information or questions that you may have. Thank you!

Pre-kindergarten

(All vaccines are needed before a child begins PK – except for Influenza – see below)

DTP (Diptheria, Tetanus, Pertussis) - 4 doses

Polio – 3 doses

MMR (measles, mumps, rubella) – 1 dose after 1st birthday

HIB (Haemophilus Influenzae type B) – 1 dose after 1st birthday

Varicella (Chicken Pox) – 1 dose after 1st birthday

Pneumococcal (PCV) – 1 dose after 1st birthday

Influenza – 1 dose between September 1st and December 31st of the current school year (deadline is December 31st)

Kindergarten

(All vaccines are needed before a child begins kindergarten)

DTP (Diptheria, Tetanus, Pertussis) – 4 doses (the last dose must be after your child’s 4th birthday – or 5 doses at any age)

Polio – 3 doses (the last dose must be after your child’s 4th birthday – or 4 doses at any age)

MMR (Measles, Mumps, Rubella) – 2 doses (1st dose after 1st birthday, the 2nd dose at least 28 days after the 1st dose)

Hepatitis B – 3 doses

Varicella (Chicken Pox) – 1 dose after the 1st birthday

Grade 6

(These vaccines are required before your child enters 6th grade in September.)

Tdap (Tetanus, diphtheria, pertussis) – 1 dose after the 10th birthday (If your child received a Tetanus shot within the past 5 years, please contact me for further information.)

Meningococcal – 1 dose after the 11th birthday

Hepatitis B vaccine – a series of 3 doses (should have received when s/he was small)

Who Needs a Physical?

Physical examinations are required by the State of New Jersey for the following student groups (see more detail below):

  1. New Students
  2. Transferring into Alloway School (only certain      situations)
  3. Recommended – at least once during each developmental stage, at early childhood (pre-school through grade three), pre-adolescence (grade four through six) and adolescence (grades seven through 12).
  4. Child Study Team Evaluation – only when indicated by the CST
  5. When a student is suspected of being under the      influence of alcohol or controlled dangerous substances
  6. Sports

 

New Students

New students who have never attended a public/private school must have a physical examination done and documentation submitted to the school nures within 30 days upon enrolling in school. (A link for the physical examination form is at the end of this page.)

 

Transferring In Students

Students transferring into Alloway Township School from a public or private school in New Jersey may need a physical examination. If documentation of a previous physical examination is provided from the sending district, a new physical will not be required. If documentation from the sending school is not provided, the school nurse will notify you and you will have 30 days to have the exam done and form submitted.

Students transferring into Alloway Township School from out-of-state or out-of-country will have 30 days to have a physical examination completed and the documentation submitted.

 

Under the Influence

When a student suspected of being under the influence of alcohol or controlled dangerous substances is reported to the certified school nurse, a physical examination must be conducted.
